How to join our dental provider networks

It’s easy to become a participating provider with us. You just need to complete and submit our online application. We have prepared a checklist to make process easy to complete. Here’s what you’ll need:

• Work history (five years)
• Education
• Hawaii State Dental License
• Certificate of Professional Liability Insurance: Must specify limits of liability—minimum $1,000,000 per claim and $3,000,000 aggregate. Copies of insurance invoices or binders are not acceptable
• DEA Certificate & State Narcotics Certificate: If unavailable, submit a signed Drug Waiver Form explaining why you don’t prescribe narcotics and who will prescribe for your patients
• Specialty Board Certificate (if applicable)
• IRS Form W-9

All documents must be current during the credentialing process.

Please complete all sections of the online application form in its entirety. Approximate length of time to complete is 10-15 minutes. Applications submitted with incomplete information will delay the credentialing process.
